What makes a transcript passage index exact?
The quoted wording should be derived from an identified contiguous segment range, stored with its start and end anchors, bound to a source fingerprint, and shown separately from editorial summary or topic labels.
Do not retype the quote
Retyping invites silent punctuation, spelling, omission, and context changes. Capture directly from the reviewed transcript segments. Write the decision in plain language and retain the original source separately so the index can be audited against something outside the browser project.
Keep anchors and text together
A locator without wording is hard to audit, while wording without a locator is hard to retrieve. Preserve both. Review the result as a retrieval aid rather than a conclusion. Human-authored fields must remain visibly distinct from exact source-derived text.
Recheck after source revision
If the transcript changes, show drift and require explicit recapture rather than quietly substituting new text. Carry the limitation into the frozen edition and passive handoff so a recipient can understand what was checked, what was not checked, and what remains their responsibility.
